The South Orange County Diabetes Collaborative is a group of health care professionals whose collaborative purpose is to: “make a positive impact on the lives of people living in South Orange County who are affected by diabetes, through their organizational and collaborative resources.” Members of the SOC Diabetes Collaborative are: Abbott Diabetes Care, American Diabetes Association, Bristol Park Medical Group, California Diabetes Prevention and Control Program, Camino Health Center, CCS Medical, Kaiser Permanente, Laguna Beach Community Clinic, Lifescan Corp., Mission Hospital, Mother’s of Children with Diabetes/Mission Lutheran Church, Roche Diagnostics, Saddleback Memorial Medical Center
